Welcome to the GLS Youth Suicide Prevention Evaluation Materials and Tools webpage!

The Garrett Lee Smith (GLS) Memorial Act Funding is a grant from the Substance Abuse and Mental Health Services Administration designed to reduce teen suicide in Nebraska. These evaluation tools are used to fully understand and improve the delivery of suicide prevention programs and the effectiveness of the activities carried out under this project. Your participation in completing the evaluation tools is essential to this process.
Please start with the Evaluation Tools Guide for information about which tools you need to complete and directions for how to complete the tools.
